Abstract

Diabetes is a chronic disease that, if not properly managed, can lead to severe complications such as cardiovascular disease, kidney failure, and neuropathy. Effective management requires lifestyle changes, medication adherence, and regular monitoring, but social determinants of health (SDOH) create barriers to comprehensive care for many underserved populations.Research highlights key challenges: Food insecurity worsens diabetes outcomes by limiting access to nutritious foods, low health literacy reduces self-management engagement, and culturally tailored interventions have been shown to improve diabetes care. Merced County's underserved populations, particularly Hispanic communities, face significant barriers such as food deserts, financial constraints, medication adherence issues, and language barriers that limit healthcare access. Analyzing and identifying diabetic outcomes and health disparities may help Golden Valley Health Care (GVHC) bridge any present gaps in care.The SMART aim of this study was to identify key social determinants of health affecting diabetes management at GVHC and propose targeted interventions that improve access to care, enhance patient education, and reduce barriers to treatment. By analyzing patient-reported data, the project sought to develop actionable solutions that improve health outcomes.
