Facts:

  • While hypertension is one of the highest prevalent diseases in the United States; control of blood pressure to an acceptable range is a shocking 45%.1
  • As of 2020, local data from Fresno Community Health Improvement Partnership (FCHIP), reports 24.1% of Fresno residents had hypertension.
  • Home blood pressure monitoring allows more frequent measurements in a familiar environment, eliminating factors that may be affecting patient's blood pressure.3

Project Objective: The goal of The Remote Patient Monitoring Blood Pressure Initiative is to assess how at home blood pressure monitoring along with adherence to the outlined program will reduce essential hypertension.

SMART AIM: Patients ages 35 to 70 diagnosed with essential hypertension in California's Central Valley that participate in The Remote Patient Monitoring Blood Pressure Initiative at OMNI Health and have high adherence in a self blood pressure monitoring treatment for four months will have at least a 10% reduction in their measured blood pressure. Adherence will be defined as patients who attend all health education visits, physician.
